Ron C. McKinnon, CSP, is an internationally experienced and acknowledged safety professional. He has been extensively involved in international research concerning the cause, effect, and control of accidental loss, near-miss reporting, accident investigation, and safety culture change for the last 38 years.
He is the author of "Cause, Effect, and Control of Accidental Loss," published by CRC Press, "Changing Safety's Paradigms," published by Government Institutes and "Safety Management: Near Miss Identification, Reporting and Investigation," published by CRC Press.
Ron holds the Diploma in Safety Management from Technikon SA, South Africa, a teaching Diploma, a Management Development Diploma (MDP) from the University of South Africa, and a M.Sc. in Safety and Health Engineering from Columbia Southern University, USA. He is a Certified Safety Professional (CSP).

My specific view of the authors view of near miss

The preface of this book is understood to provide an endorsement of H.W. Heinrich’s safety theories of some 80 years ago. This is a difficult endorsement to understand in 2015 with the amount of data that has been collected by the United States government and published. Such currently published data does not appear to support Heinrich’s theory of a ratio between mishaps and near-miss. In Professional Safety, October 2011, Fred Manuele published a review of Heinrich’s theory and the data titled "Reviewing Heinrich Dislodging Two Myths From the Practice of Safety." Fred Manuele concludes that knowledge has evolved about how accidents occur and their causal factors, the emphasis is now properly placed on improving the work system, rather than on worker behavior. The author of this book makes no attempt answer such challenges to Heinrich but instead appears to embrace the theories as fact and leverages them to support the book.
The work also appears to use the concept of “Luck” but this is not the most precise term. The idea that good or bad fortune controls ones fate is a bit difficult to embrace. It would have been perhaps more precise to have described the near miss as an outcome with a probability. The work appear to also build upon the idea that root cause actual exists. While there is a method of root cause analysis, it is unclear if anyone has ever completed one. That is, when one thinks about the term “root cause” it can be somewhat difficult to definitively prove that singular cause.
Near miss is not so refined an area of research or work that only one view or model exists. There was small consideration for the merits of other views. For example the National Academies Press, in 2004 provided the "Accident Precursor Analysis and Management: Reducing Technology Risk Through Diligence" which is an interesting discussion of the near miss topic in general and it is still understood to be a free resource.
However, this author does provide a disciplined approach to the topic of near miss regardless of our areas of disagreement and as well a reasonable introduction to the topic of near-miss.
